Positions and Descriptions

Registered Nurse (RN)

Registered Nurses maintain standards of professional nursing practice in the home care setting under the direction of the Clinical Manager. They help patients with different diagnoses and needs, such as elderly, critically ill, disabled, recovering, or palliative care patients. They perform tasks such as administering IVs, changing dressings, cleaning wounds, collecting blood samples, monitoring vital signs, and responding to emergencies. They also update doctors about their patients’ health and teach patients about their diagnosis and treatment plans.

Physical Therapist (PT)

Physical Therapist organizes and conducts medically prescribed physical therapy programs to restore function, prevent disability following disease, injury, or loss of a body part, and helps patients reach maximum performance levels. Evaluates, records and reports on patient progress. In addition, physical therapists are required to perform comprehensive at-home evaluations and maintain an open line of communication with patients, family members and caregivers.

Occupational Therapist (OT)

Occupational Therapist work to adapt the home environment to help people live better with disabilities, injuries, or illness. In addition, occupational therapists are required to perform comprehensive at-home evaluations and maintain an open line of communication with patients, family members and caregivers.

Physical Therapist Assistant (PTA)

Physical therapist assistants provide physical therapist services under the direction and supervision of a Physical Therapist. PTAs implement the components of patient care, obtain data related patient treatment, and work with the PT to modify care as necessary.

Occupational Therapy Assistant (OTA)

Occupational Therapy Assistants provide occupational therapy services under the direction and supervision of an Occupational Therapist. They develop, recover, improve, as well as maintain the skills needed for daily living and working. They also assist in training family members and Aides in training to promote patient recovery.

Medical Social Worker (MSW)

Medical Social Workers help patients and their families cope with illnesses and injuries. They provide them with information and resources to deal with the emotional, financial, and social needs that arise with a diagnosis. They may assist clients with specific challenges, such as neglect, abuse, domestic violence, substance abuse, or terminal illness.

Speech Language Pathologist (SLP)

Speech Language Pathologist works with patients to improve communication skills. They work with patients with developmental disorders, neurological conditions, or injuries that prevent effective communication or consuming food or drinks safely.

A Certified Nursing Assistant (CNA)

Certified Nursing Assistants assists patients with daily living and provides basic care. They work under the supervision of licensed Registered Nurses (RN). CNAs perform a variety of tasks such as measuring and recording vital signs, assisting with in-home mobility equipment, helping with meals, and eating, assisting with getting dressed and medication reminders. CNAs must pass a state exam be certified with New Mexico state.

Home Health Aides (HHA)

Home Health Aides also assist patients with daily living and provide basic care. They work under the supervision of licensed Registered Nurses (RN). HHAs perform a variety of tasks such as measuring and recording vital signs, assisting with in-home mobility equipment, helping with meals, and eating, assisting with getting dressed and medication reminders. HHA are not required to be certified with the state of New Mexico, however, do meet strict75 hour training requirement.

Homemaker/Personal Care Attendant (HM/PCA)

Homemakers/Personal Care Attendants help with patient in their home with mobility form one location to another. They assist with grooming, dressing, cooking, and cleaning. They also do medication reminders and provide general companionship.
